Lead Qualification Form: AI Scores Every Inquiry Before You Call

A contact form collects names and emails. A lead qualification form tells you which leads are worth your time before you pick up the phone. This guide builds an AI-powered qualification form that scores leads automatically and routes hot prospects directly to your calendar.


Before You Start

You’ll need:

  • Familiar with AIWU forms (Create Your First Smart Form)
  • AIWU Pro with API key configured
  • Clear criteria for what makes a qualified lead for your business
  • Time needed: ~20 minutes
  • Plan required: Pro

Step 1: Define Your Qualification Criteria

Before building the form, define what “qualified” means for your business. Common B2B criteria:

  • Budget: Can they afford your service? (e.g. minimum project value $5,000)
  • Authority: Are they the decision maker?
  • Need: Do they have the problem you solve?
  • Timeline: Are they ready to move in the next 30–90 days?

Your form questions should uncover this data naturally, without feeling like an interrogation.


Step 2: Build the Form Fields

Go to AI Copilot → Forms & Calculators → Create New Form. Name it: “Project Inquiry”.

Add these fields:

Step 3: Write the AI Qualification Prompt

In the form’s AI Action settings, add a prompt that runs on submission:

Analyze this sales inquiry and produce a lead qualification report.

Submission data:
- Name: {field_name}
- Company: {field_company}
- Project description: {field_project_description}
- Budget: {field_budget}
- Timeline: {field_timeline}
- Role: {field_role}

Our ideal client: [describe your ideal client — industry, company size, problem they have, budget they typically have]

Score this lead on a scale of 1–10 (10 = perfect fit, 1 = not a fit) based on:
- Budget alignment with our typical project range ($5k–$50k)
- Timeline urgency
- Decision-maker status (are they likely the buyer?)
- Problem fit (does their description match what we do?)

Return your response in this format:
SCORE: [1–10]
TIER: [Hot / Warm / Cold]
SUMMARY: [2 sentences on why this score]
RECOMMENDED ACTION: [Call within 24h / Send proposal template / Nurture sequence / Politely decline]
RED FLAGS: [Any concerns, or "None"]


Step 4: Configure What Happens on Submission

Set up routing based on the AI qualification result:

For Hot leads (score 7–10):

  • Email notification to you with full qualification report
  • Auto-reply to the lead with a calendar booking link
  • Optional: Slack notification with “🔥 Hot Lead” alert

For Warm leads (score 4–6):

  • Email notification to you (lower priority)
  • Auto-reply with a case study or portfolio link

For Cold leads (score 1–3):

  • Auto-reply thanking them and suggesting a self-serve resource
  • No internal notification — review weekly instead of immediately

Configure this routing in the form’s Conditional Actions settings using the SCORE or TIER from the AI output.


Step 5: Write the Auto-Reply Emails

Hot lead auto-reply:

Write a warm, personalized reply to {field_name} who just submitted a project inquiry on our website. They're looking for help with: {field_project_description}.

Tell them:
1. We've reviewed their inquiry and it looks like a great fit
2. We'd love to schedule a 20-minute discovery call
3. They can book directly at: [your calendar link]
4. Or reply to this email with questions

Tone: enthusiastic but professional. Max 80 words. Sign off from [your name].


Verify It’s Working

  1. Submit a test inquiry using your own email
  2. Check that the AI qualification report arrives in your inbox
  3. Verify the SCORE and TIER are parsed correctly
  4. Check that the auto-reply arrived in the submitter’s inbox
